- Enrollment Verification and Arrival Report

This is the most important form you will need while abroad. It is due within
the first five days after classes begin at your host university for each semester.
You must have it completed and faxed no sooner than the first day of classes
and no later than the fifth day of classes.
Without this form, your enrollment can not be verified. If you are receiving
federal financial aid, your aid cannot be released until your enrollment is
verified.
For each course listed on the Enrollment Verification and Arrival Report
which was not previously evaluated on the Course Equivalency form, a description
of the course, the level in the host university degree plan in which the course
is taught, and the number of credits for the course must
be faxed with the Enrollment Verification.
Once all this information is received, we will be able to have the course(s)
evaluated and your advisor will be able to make a determination as to how the
course equivalent(s) applies to your degree plan at OSU.
If you are receiving federal financial aid, it is at this point your financial
aid will be released.
- Add/Drop Notification

This form is to be used only if you make any changes to the schedule you
reported on your Enrollment Verification. The concept of this form is the same
as the Add/Drop card used by OSU.
You may make changes to your schedule with no effect on your transcript up
to six weeks after your host courses begin. After six weeks, the only courses
that will appear on your OSU transcript are those in which you are enrolled
after six weeks.
After six weeks, you may NOT add any courses. However, you may withdraw from
courses with a grade of “W” posted to your OSU transcript.
- Academic Progress Report

Please complete this form if you are a recipient of financial aid (including
scholarships), are studying abroad for more than one semester, quarter, or block,
and are unable to provide an official transcript of your grades at the end of
your first period of study.
OSU cannot disburse your financial aid for the subsequent academic term until
you submit either the grades for the completed coursework (official transcript)
or, if grades are unavailable, a statement signed by an appropriate official
or faculty persons at your host institution, verifying that you are making satisfactory
progress.
Often times, it is faster to have this form completed and submitted so financial
aid can be released. For this reason, it is recommended that this form be completed
at the end of each semester, even if you are studying abroad for only one semester.
This does not negate the need for an official transcript, however.
